Weight Management Program (“WMP”) Terms & Conditions

OVERVIEW

The Weight Management Program ("WMP") is designed to help users achieve their weight management goals through personalized plans, ongoing support and monitoring, and a focus on sustainable lifestyle changes. Our team of healthcare professionals will work with you to create a plan that is tailored to your individual needs and goals, including dietary recommendations, exercise routines, and other lifestyle modifications as needed.

By starting the Weight Management Program, you acknowledge and agree to these terms and conditions below:

COMMITMENT

By enrolling in the Weight Management Program offered by MaNaDr, you acknowledge that achieving your weight management goals requires commitment to making lifestyle changes. This includes following our recommended dietary and exercise plans, making other lifestyle modifications as needed, and actively participating in the program.

SERVICES

a) Personalized Plan: Our healthcare professionals will work with you to create a personalized plan that is tailored to your individual needs and goals. This may include dietary recommendations, exercise routines, and other lifestyle modifications to help you achieve your desired results.

b) Support: Throughout the program, you will receive ongoing support and guidance from our team of trained professionals. This may include regular check-ins, counseling, and other forms of support as needed.

c) Monitoring: We will regularly monitor your progress throughout the program to ensure that you are on track to achieve your goals. This may include regular weigh-ins, body composition measurements, and other forms of monitoring as needed.

d) Accountability: We expect you to take responsibility for your progress throughout the program. This includes following the personalized plan created for you, attending all scheduled appointments, and actively participating in the program.

e) Duration: The Weight Management Program is designed to provide a realistic timeline for achieving your weight management goals while emphasizing sustainable lifestyle changes for long-term success. Our team of professionals will work with you to create a plan that is both achievable and sustainable for your individual needs.

f) Safety: Our team of professionals prioritizes your safety and health throughout the program. Any dietary or exercise recommendations will be safe and appropriate for your individual needs, and we will work with you to ensure that you are comfortable with all recommendations.

g) Accessibility: We strive to make our Weight Management Program accessible and affordable for individuals from different backgrounds. We offer a range of payment options and will work with you to find a plan that fits your budget.

COMPLIANCE

By enrolling in the program, you agree to comply with all program requirements and guidelines, including attending scheduled appointments, adhering to dietary and exercise plans, and reporting any health issues or concerns to your assigned Weight Management Provider.

TERMINATION

As a user, you have the right to opt out of the weight management program at any time.

a) If you wish to opt out of the program, you must notify your healthcare provider in writing or through the platform's messaging system.

b) If you have already made payment for the program, you may be eligible for a refund, depending on the terms and conditions of your payment agreement.

c) Refunds, if applicable, will be processed within a reasonable period of time, subject to the terms and conditions of your payment agreement.

d) If you choose to opt out of the program, you understand that you will not receive further guidance or support from our team of professionals, and you will be responsible for managing your weight and health on your own.

e) You acknowledge that the weight management program is designed to provide you with guidance, support, and motivation to help you achieve your weight management goals, and that opting out of the program may hinder your ability to achieve these goals.

f) You agree to hold harmless the weight management program, its owners, employees, agents, and affiliates from any claims, damages, or losses that may arise as a result of your decision to opt out of the program.

g) MaNaDr reserves the right to decline or terminate your participation in the program if, in our sole discretion, we determine that the program is not suitable for you or may be harmful to your health.

h) The weight management program reserves the right to modify, amend, or terminate the program at any time, with or without notice to you.

NO GUARANTEE

Please note that the program is not a guarantee of weight loss or any specific health outcome, and results may vary based on individual circumstances and adherence to program guidelines.
